Funeral Home acknowledges the importance of privacy to you personally
and that of your family. Our obligation, professionally and legally,
is to keep any personal information we received from you and your
family completely confidential.
The information
we collect is needed to identify the deceased/beneficiary, for
at-need and pre-need funeral arrangements. The purpose is to identify
the deceased/beneficiary, the purchaser, the next kin, the executor(s)
and/or power of attorney of the deceased/beneficiary and to provide
services requested by you. All this information is required by
our funeral home to be able to meet your needs and comply with
all legislative and regulatory requirements.
Information
is only shared with third parties when required by law, or if
you personally authorize us to, or if it is required for the provision
to you, a service or product supplied by said third party. This
would be any cash disbursements or phone calls you have authorized
us to make on your behalf. Information must be shared with government
agencies, vault companies, cemeteries, crematoriums, clergy, churches,
monument companies, coroner/doctor, obit/death notices and any
other contracted services, as part of our service to you. Please
be advised that Death Registration and Funeral Director Certificates
of Death are required by law. Also, for your convenience, we post
on our web-site, the funeral notice, unless specific instructions
by you not to post said notice is received in advance. We do not
forward the funeral account, or copy of said funeral account,
to any other person, legal firm, insurance company, etc, for payment,
unless we receive written instructions from you.
You
may have access to this personal information we hold about you,
for the purpose of review, update or correction. We reserve the
right to confirm your identity and legal right, before complying
with the request. We require a letter in writing, from you, stating
your request and information requested, prior to us complying
to your request. If you require any information about a deceased
person, you must provide legal proof, such as a will, showing
proof, you are the executor and legal representative of said deceased
person. If we are unable to provide you with access to all of
the personal information we have about you or about the person
you request, then a reasonable explanation will be provided.
Reasonable precaution is taken to ensure this personal information
is correct and kept safe from loss/theft, unauthorized access
or disclosure. We do not sell, trade or give away any of this
information you have entrusted to us.
